The placement of module items on form pages is related to Itemid Variable in the URL.

The simple way to add modules to a form page is this:

  1. Add a new content item to the main menu - doesnt matter what content you link this item to. 1a. Give this new ite in the main menu a name you can remember well - like "Contact Form Fix"
  2. UNPUBLISH that new link frm the main menu
  3. Note the ITEMID of that item.
  4. Now when you add the link to the main menu for the Phil-a-form form simply change Itemid=0 to replace the 0 with the ITEMID of the menu link you unpublished.
  5. Now when you assign modules to "Contact Form Fix" menu item they will appear on the form page.

This solution can also be applied to have a different template assigned for the form.
